Village Pediatrics accepts the following insurance plans:

Please bring a copy of your child's insurance card to your visit.
Insurance
You are responsible for providing us with up-to-date insurance information.  If you have changed insurances, you must provide a copy of your new insurance card.
Your health insurance is a contract between you and your insurance company.  Village Pediatrics is not a party to that contract.  It is your responsibilty to know and understand the details of your insurance coverage.  You may be held liable for any services that your insurance carrier does not pay.
Patient Payment
Co-payments will be collected at the time of your appointment.  We will add a $5.00 surcharge to any unpaid co-payment that is billed after the date of service.
You must pay any deductible, co-insurance or other patient balance incurred due to non-payment by your insurance within 30 days of receiving a bill from Village Pediatrics.
If you have not paid your balance after 90 days from the date of the original bill, your account will be considered delinquent and may be turned over to a collection agency.
No Show Fee
You will be charged a No Show Fee for any missed appointment where you fail to notify us at least 24 hours beforehand.
